The claim

“Most people should take fish oil or omega-3 supplements.”

Mostly false
Evidence confidence Moderate

Moderate confidence because a large primary-prevention trial found that a standard 1 g/day fish-oil supplement did not reduce the primary composite cardiovascular outcome in generally healthy older adults. Prescription omega-3 products have specific triglyceride and cardiovascular indications that should not be generalized to ordinary supplements.

The facts

Most people do not need a fish-oil supplement for general heart or cancer prevention. Omega-3 treatment can be useful for specific indications, and eating seafood provides nutrients that capsules do not fully replace.

The bigger picture

“Omega-3” covers different products and doses. Over-the-counter EPA/DHA mixtures are not equivalent to prescription icosapent ethyl. REDUCE-IT’s cardiovascular benefit occurred with 4 g/day prescription EPA in statin-treated, high-risk patients with elevated triglycerides, not the general population.

Who should be careful?

Safety note: High-dose omega-3 treatment can increase atrial-fibrillation risk and may affect bleeding or gastrointestinal symptoms. Product purity and labeled dose vary among supplements. Do not substitute a retail fish-oil product for prescribed icosapent ethyl.

Talk to a healthcare professional if: Discuss omega-3 treatment if you have very high triglycerides or meet a prescription indication. Ask before high doses if you have atrial fibrillation, a bleeding disorder, take anticoagulants or antiplatelet medicines, or have surgery planned.

What this means for you

For most people, food is the default and a capsule is optional rather than required. A prescription product may be appropriate for a defined medical reason.

Your next step

Aim for seafood within a balanced diet if you eat it. If considering a supplement, identify the goal, EPA/DHA dose and whether evidence exists for your situation.
