The claim

“Testosterone therapy is safe and worth it for most men with low testosterone.”

It depends
Evidence confidence Strong

Strong confidence for the conditional conclusion. Guidelines require symptoms plus consistently low morning testosterone, and a large randomized trial was reassuring for major cardiovascular events in appropriately selected men. Benefits, fertility effects, monitoring and longer-term uncertainties still vary by person.

The facts

Testosterone can improve selected symptoms in men with confirmed hypogonadism, but one low test or nonspecific fatigue is not enough. Whether treatment is worthwhile depends on the cause, symptoms, fertility goals, contraindications and response.

The bigger picture

The strongest safety trial followed men for about three years and was designed around major cardiovascular events. It does not prove lifelong safety or justify use for normal aging, bodybuilding or an unconfirmed laboratory result.

Who should be careful?

Safety note: Testosterone suppresses sperm production, can raise hematocrit and requires monitoring. In TRAVERSE, major cardiovascular events were not increased, but atrial fibrillation, acute kidney injury and pulmonary embolism occurred more often in the testosterone group.

Talk to a healthcare professional if: See a clinician before treatment, especially if you want fertility or have elevated hematocrit, untreated severe sleep apnea, prostate or breast cancer concerns, severe urinary symptoms, recent heart attack or stroke, uncontrolled heart failure or thrombophilia.

What this means for you

A careful diagnosis and a measurable treatment goal matter more than simply moving a number into the reference range.

Your next step

Repeat an early-morning fasting testosterone test with a reliable assay, evaluate the cause, and discuss fertility and monitoring before starting therapy.
